His love for his wife, Mya, and daughter, Natalie, causes his distress until 18 years later. Back then, Mya wished for him to never hold guns or sacrifice anyone in the brutal fight. Mya also wishes for him to be a good father to Natalie.

Part 3: What Happens To Isaac Kane And Natalie Kane In The Return Of The Reaper Ending?
Return Of The Reaper Ending portrays the peak of Isaac Kane And Natalie Kane’s quest. At this point, Isaac has killed Cassius, another assassin in his guild, who underestimates him in a battle. By doing so, he has convinced the assassins of his capability as Chernobog.
The previous episodes have Cassius and the entire assassins calling him a weak sheep. That’s the same name he encounters when he’s trapped in the Russian mobsters’ dungeon! Plus, at this point, the assassins didn’t believe Isaac, even when he held the Reaper Amulet.
Now, Isaac has strengthened his position as the assassin’s heir and gained trust amongst assassins. However, his quest to take down the mobsters is not over yet. That’s why a female assassin invites him to try out the weapons to defeat the Russian mobsters.
After all, they still hold Natalie hostage, and the ending even features them about to shoot Natalie’s head. Little do the mobsters know that Isaac has the bullet that can bend around and use the Dragon Breath Technique.
Isaac eventually aims the bullet at Igor, who recently killed his father and became the mobster’s heir. Isaac does it after he asks Igor a rhetorical question about the tea.
Now that Isaac’s death toll has increased, he not only gets more tattoos. Instead, he visits his late wife’s grave while drinking his last bottle of alcoholic beverage.
We can witness Isaac’s regret when he does it. Yet, the ghost of his wife, who later transforms into the real-world Natalie, convinces him. No matter how many souls he has sacrificed, he is still Mya’s husband and Natalie’s father!
